RICTAT Approved Client Questionnaire & Consent Forms
As of the 1st January 2013, all RICTAT registered therapists will be required to use the RICTAT Approved Consent Form (download the forms from the links below) when accepting new clients. We would hope that within 6 months, all repeat clients have completed the new RICTAT Approved Consent Forms.
We would also recommend using the Intake Questionnaire, Aftercare Advice, Continued Education Record Form & Best Practice Guide as good practice.
